The process of becoming a patient at the Integrative clinic involves:

The first step is filling out the intake form, which is about 40 pages long. Your physician needs to know about your lifestyle, including what you eat, your hobbies, medical history, family history and more. This allows your physician to order the appropriate labs. Your first visit will be after the initial lab results are ready. The first visit usually takes 2-3 hours. The fees for appointments: Initial visit: $849 , Any follow up visits: $424. The initial fee is due before reviewing the intake form. The fee for the labs should be paid to each lab directly and is not included in the provider’s fee. There are labs that are not covered by any insurance company and the patient is responsible to pay for those when needed.
