What Are A 4D / 7D Facelift with Fotona Laser?
The 7D Facelift is a multi-step laser treatment that addresses aging from the inside and outside of the skin. They are completely non-invasive treatments that stimulate new collagen synthesis (neocollagenesis), to reduce the appearance of sagging skin and wrinkles. These treatments also improve tone and texture, and treat sun-damaged skin, without injectables, while being safe for all skin types.
Both options are performed with the two laser wavelengths Er:YAG and Nd:YAG, in a series of steps with a comprehensive approach to rejuvenation.

How the Treatment Works
The 7D is similar to the 4D Facelift but addresses a few more areas of the face, with four more treatments included.
The 4D steps are:
- Step 1: Intraoral tightening. The handpiece is inserted into the mouth to treat marionette lines from the inside. Fotona is the only laser that offers treatment from inside the mouth.
- Step 2: Pigment and texture (FRAC3®). Fractional skin resurfacing.
- Step 3: Skin tightening (PIANO®) – The goal of step three is to bring the skin to the optimal temperature for skin tightening.
- Step 4: Light peel – the depth and effectiveness can be treated to align with your downtime.
7D also includes:
- SmoothEye® (eyelid tightening)
- Vector Lift® (eyebrow lift)
- Liplase®
- Submental (under the chin). Specifically, it addresses sagging skin and definition loss in the lower face and neck. This treatment produces a better definition of the jawline.

What to expect
The first step is the consultation and exploring options.
Then all patients need to be on a prescription topical cream through a compounding pharmacy for 1-2 months ( depending on the skin type) before the first session.
Many of our patients continue using the topical cream as their night cream even after they finish all sessions.
The cream contains: tranexamic acid, vitamin C, Kojic acid and niacinamide, sometimes hydroquinone added for a short period of time. The percentage of each ingredient is different in different patients, depending on their skin condition.
At the first treatment session, we apply the strongest topical cream (BLT cream) and wait for 30 min, then start the treatment.
Downtime depends on the patient’s choice of treatment during the last step which is the peel.
You may choose the deep peel with actual peeling and downtime from 3-7 days. The second option is choosing the T runner for the last step without any downtime or peeling. I recommend that every patient have at least one peel.

Does a Laser Facelift eliminate the need for neurotoxins?
Neurotoxins such as Botox are mainly for prevention and not treatment. There are two types of wrinkles on the face:
- Dynamic lines: The ones we see when we move muscles, such as the appearance of frown lines (the 11s) in between eyebrows when frowning or crow’s feet when smiling, but these lines disappear when we stop moving muscles.
- Static lines: When there is collagen loss and the wrinkles and lines are there even when not moving any muscles.
Botox or other neurotoxins help to remove dynamic lines and prevent static lines.
Therefore, even after the Fotona facelift and building collagen and reducing wrinkles, you may still need the neurotoxins for prevention.
How long does it take?
Typically, it is recommended to do 1 session per decade of life. For example, a 40-year-old typically needs 4 treatments. However, each patient will receive a specific protocol of sessions based upon needs and condition of skin.
Complete results take place gradually, over 2 – 3 months. Continuing improvement for up to six months.
Treatment sessions are performed at 4-6-week intervals for a total of 3-7 consecutive sessions. Example: Patient in their sixties, as an initial package of 6 sessions recommended, followed by yearly maintenance treatment.
Maintenance treatments: One session each year.

Preparation & Care for Treatment
Before Treatment
2 Weeks Prior:
- Avoid sun-exposure, tanning beds, or sunlamps. Patients are unable to be treated with a tan or a sunburn on the face.
- Please do not use perfume, deodorant, waxing or chemical peel on the treatment area for two weeks prior to the treatment.
- Prior to Arrival Make sure the face is thoroughly cleansed. All make up must be removed. For men, face skin must be clean and fresh shaved.
After Treatment
General:
- Apply a re-epithelizing cream as often as needed to improve the healing process. Cream recommendations to select from are: Bariederm Cicacream (Uriage) Cicaplast (La Roche Posay) Cicalfate (Avenne) Aquaphor (Eucerin).
- Apply broad spectrum sunscreen, 3 times a day (7 am, 12 pm and 2 pm).
- Immediately After : Can apply make-up (but best to wait 24 hours). • After 4-5 Days : Topical treatments and skin care dermo-cosmetic products, such as retinol and acids can be resumed.
- For 7 Days Strict avoidance of sunlight. Erythema should be expected immediately after the session lasting less than 24 hours, followed by a possible superficial feeling over the next 48-72 hours.
- Immediately after the treatment, freshness and tightness of the treated skin. Complete results take place gradually, over a time period of 2-3 months. Continuing improvement for up to six months.
