What is Fractional Laser resurfacing?

Your treatment at Pristine Wellness Center

Before treatment

Dr. Izadi evaluates your skin type, pigmentation history, medications, and previous treatments. You will receive individualized instructions about skin preparation, sun protection, and products to avoid.
Some patients need a period of skin preparation before treatment.
Every patient who needs laser treatment, should apply a compounded cream on the area every night for at least one month in order to prevent post inflammatory hyperpigmentation ( PIH) which is the darkening of the skin ( might happen with any energy device). The compounded cream: Tranexamic acid, Niacinamide and Azelaic acid.
Before the visit for the laser treatment, please wash your face, remove all jewelries, no make up and wear a comfortable clothes with open front so you can remove it at home after the treatment without rubbing it on your skin.

5-7 days prior to peel:1. Inform the doctor if you have taken Accutane ( oral acne medication) in the past 3 months.2. Stop application of al topical retinoids and vitamin A derivative products3. Stop use of all glycolic or chemical exfoliants4. Avoid facial waxing and tweezing5. Avoid excessive sun exposure and tanning beds6. If you have a history of cold sores, your physician will prescribe acyclovir ( oral antiviral medication) please start the medication 24hours prior to the peel and continue the course after the peel.

During treatment

The laser is applied to the selected treatment areas using settings tailored to your needs. Comfort measures and treatment duration depend on the area treated and the intensity of resurfacing.

After treatment

Redness, swelling, sensitivity, and peeling may occur. Recovery varies with treatment depth and your skin’s response; fractional resurfacing should not be assumed to have no downtime.
Following your aftercare instructions and protecting your skin from the sun are important for healing and maintaining results.
Here is the specific care after any peeling:
Post peel skin care:
1. Avoid direct sunlight on the treated area and apply broad spectrum sunscreen SPF of 40 and above daily and reapply every 2 hours.
2. Keep your skin well hydrated, moisturizing liberally and frequently with a protective barrier such as aquaphor
3. Wash your face with a gentle cleanser, cool water, using hands only and pat dry 4 hours after the peel.
4. Apply diluted vinegar soaks with gauze or clean hand towel : 1tablespoon of white vinegar diluted in one cup of cold water (you may add ice cubes as well) apply the soak for 20-30 min at a time, two times a day and start 24 hours after the procedure. This soak should be soothing and not causing burning.
5. If any skin care products cause stinging or burning, rinse of and try again in 48-72 hours
6. Avoid using abrasive products such as scrubs or exfoliating cleansers for 7-10 days
7. Avoid using topical retinoids or glycolic acid-containing products for 7-10 days
8. Vitamin C antioxidant serums may be resumed 48 hours after the peel if not causing burning or stinging
9. Avoid strenuous exercise or sweating for 24 hours.
YOU DON'T HAVE TO COMPLETELY REMOVE THE AQUAPHOR FROM YOUR SKIN DURING THE SHOWER, DO NOT RUB OR SCRUB YOUR SKIN IN THE SHOWER UNTIL YOU COMPLETE THE PEEL.

When will I see the results?

AbstractBackground: Fractional photothermolysis is a modern resurfacing technique, in which microscopic zones of thermal injury are created, stimulating turnover of both epidermis and dermis. Fractional laser rejuvenation has been developed to overcome the drawbacks of traditional ablative laser.

Objectives: To objectively evaluate the effectiveness of multiple sessions of fractional Er:YAG laser rejuvenation for aging upper face clinically, histologically and immunohistochemically.

Patients/methods: Ten volunteers asking for facial rejuvenation were subjected to multiple sessions (3-5) of fractional Er:YAG laser. Clinical evaluation with both histopathological and immunohistochemical assessment for skin biopsies was carried out before, after 1 month and 6 months of laser resurfacing. Histometry for epidermal thickness and quantitative assessment for neocollagen formation, collagen I, III, and VII, elastin and tropoelastin were carried out for all skin biopsies.

Results: Comparing before, after 1 month and 6 months of fractional Er:YAG laser resurfacing resulted in improved clinical appearance with increased epidermal thickness (P < 0.001). Dermal collagen showed increased neocollagen formation (P = 0.006), with increased concentration of collagen types I (P < 0.001), III (P < 0.001), and VII (P = 0.001). Dermal elastic tissue studies revealed decreased elastin, while tropoelastin concentration increased after laser resurfacing (P < 0.001). An increase in collagen (I and III) and tropoelastin level and decreased elastin content was encountered with increasing the number of sessions, yet it was not significant.

Conclusions: Multiple sessions are effective in rejuvenation of the aging face with high safety, short downtime, and no adverse effects. They stimulated formation of new collagen (type I, III, and VII) up to 6 months after treatment with better improvement in skin texture and fine wrinkles. The variable number of fractional Er:YAG laser sessions (3-5) showed no significant difference as regards efficacy.
